I find it confusing as well.
I think it's just the way each syllable is said/the emotion in each syllable?
Like for a sweet append I'd want to be all soft and kawaii-desu-ne.

yeah, basically. It's just a voicebank of the same voice but in a different style. Imo, the only really critical append is soft, if you have a loud character. Neko's soft VCV's are ABSOLUTELY HEAVENLY. A lot of the other types aren't that useful (dark, perhaps, if you can pull it off).
